It is the highest mountain outside the Himalayan range, reaching nearly 7,000 m in altitude and, in a way, relatively close to Brazil. Both experienced mountaineers looking to reach new summits and newcomers eager to experience a high-altitude mountain embark on this journey. The problem is that many of them do not fully understand what this means and what truly awaits them. In simple terms: breathing becomes difficult. By undergoing a process called acclimatization, where the body gradually adapts to the lack of oxygen. Each person has their own acclimatization time, and this process can take days or even weeks, depending on the target altitude and individual physiology.<\/p>\n<p>However, in this article, we will not discuss acclimatization but rather what happens when our body starts feeling the effects of oxygen deprivation or when acclimatization does not occur as expected, leading to <strong>Acute Mountain Sickness (AMS)<\/strong>\u2014from its mildest forms to severe conditions that can be fatal.<\/p>\n<h2>What Is Acute Mountain Sickness (AMS)?<\/h2>\n<p>There is a significant debate about what qualifies as a high-altitude mountain. <strong>It is common in the mountaineering community to refer to altitude when above 4,000 m, but for our body, 3,000 m already represents <a href=\"https:\/\/geartips.club\/plus\/filme_video\/fisiologia-de-alta-montanha\/\" target=\"_blank\" title=\"Online lecture on High-Altitude Physiology\">physiological changes<\/a>.<\/strong><\/p>\n<p>Some people are more sensitive and may experience symptoms such as headaches and nausea at just 2,500 m\u2014these are typical signs of Acute Mountain Sickness (AMS). The incidence of AMS varies depending on the logistics of the climb (faster ascents tend to have a higher incidence, both of mild and severe cases). <strong>The highest incidence occurs between 3,500 m and 4,500 m, perhaps because this altitude is quickly reached by climbers before beginning the acclimatization process.<\/strong> In the French Alps, for example, there is a high incidence of Acute Mountain Sickness, as mountains between 4,000 m and 5,000 m are often climbed very quickly.<\/p>\n<h2>Symptoms of Altitude Sickness<\/h2>\n<p>Altitude sickness is a collection of symptoms that typically appear hours or days after reaching high altitude as a response to the body&#8217;s adaptation to oxygen deprivation. Its diagnosis is clinical, based on symptoms. The intensity and number of symptoms vary from person to person, and the most common include:<\/p>\n<p><!--more--><\/p>\n<ul>\n<li>Headache;<\/li>\n<li>Weakness and\/or fatigue;<\/li>\n<li>Nausea, vomiting, or loss of appetite;<\/li>\n<li>Dizziness or vertigo;<\/li>\n<li>Insomnia or a feeling of suffocation at night.<\/li>\n<\/ul>\n<h2>Risk Factors for Developing AMS<\/h2>\n<p>According to the <a href=\"https:\/\/www.theuiaa.org\/\" target=\"_blank\" title=\"UIAA Website\" rel=\"noopener\">UIAA (Union Internationale des Associations d\u00b4Alpinisme \u2013 International Climbing and Mountaineering Federation)<\/a>, there are several risk factors for developing AMS (Acute Mountain Sickness) and its severe forms, High-Altitude Pulmonary Edema (HAPE) and High-Altitude Cerebral Edema (HACE). These include:<\/p>\n<ul>\n<li>Ignorance of the need for acclimatization;<\/li>\n<li>Rapid ascent;<\/li>\n<li>Previous history of AMS, HAPE, or HACE;<\/li>\n<li>Ignoring initial symptoms;<\/li>\n<li>Dehydration;<\/li>\n<li>Age (people over 65 are three times more likely to develop HAPE). Although the table is recommended for researchers and scholars rather than as a diagnostic tool, it is widely used by mountain guides, even though it does not replace a proper clinical evaluation.<\/strong><\/p>\n<h2>Treatment for Acute Mountain Sickness<\/h2>\n<p>The primary focus of treatment is symptom resolution. One of the drugs used for both prevention and treatment of Acute Mountain Sickness\u2014well known among high-altitude practitioners and even considered doping by some\u2014is Diamox (whose active ingredient is Acetazolamide). However, it is merely a diuretic that has the side effect of acidifying the blood, causing increased breathing to balance blood pH. This means that by breathing faster, we take in more oxygen, whether during the day or at night (when respiratory rates decrease during sleep, leading to more hypoxia and worsening symptoms\u2014hence the recommendation to &#8220;climb high and sleep low&#8221;).<\/p>\n<p>Since Acetazolamide is a diuretic (a medication that increases urine output), it is essential to increase hydration, which is crucial for both preventing and improving AMS and its severe forms. However, if symptoms worsen or the person continues to ascend without allowing proper adaptation, severe conditions may develop, such as High-Altitude Pulmonary Edema (HAPE) and High-Altitude Cerebral Edema (HACE).<\/p>\n<h3>High-Altitude Pulmonary Edema (HAPE)<\/h3>\n<p><strong>High-Altitude Pulmonary Edema<\/strong> is not caused by heart failure and has a different mechanism from hypertensive pulmonary edema, though its exact causes are still not well understood.<\/p>\n<p>The main variables\u2014both controllable and uncontrollable\u2014that predispose a person to HAPE include genetic predisposition, maximum overnight altitude, ascent rate, physical exhaustion, body exposure to cold, and recent exposure to high altitudes.<br \/>\nIt is a serious condition and should be treated as an emergency. The main symptoms include decreased exercise performance, respiratory fatigue even at rest, and a dry cough. It progresses with worsening shortness of breath, extreme fatigue, and a productive cough with or without bloody sputum. Fever, cyanosis (bluish lips and fingers), tachycardia, low oxygen saturation, and even loss of consciousness may occur.<\/p>\n<p>Treatment involves providing supplemental oxygen and descending as soon as possible. Proper care of patients experiencing consciousness alterations\u2014or those unconscious\u2014is crucial.<\/p>\n<h2>The Importance of Acclimatization and Prevention<\/h2>\n<p>Despite its relatively simple treatment and good prognosis, Acute Mountain Sickness can disrupt an expedition\u2019s progress, logistics, or even force its cancellation. Severe cases can put not only the expedition at risk but also the participant&#8217;s life. You may climb higher during the day but should return to a lower altitude to sleep;<\/li>\n<li>Climb high, sleep low;<\/li>\n<li>The use of medications to prevent altitude sickness should be reserved for special situations, such as rapid ascents or individuals with known acclimatization difficulties.<\/li>\n<\/ol>\n<h2>Conclusion: Safety in High-Altitude Mountaineering<\/h2>\n<p>To have a safe and more comfortable experience in high-altitude mountaineering, it is essential to understand acclimatization and recognize, prevent, and treat Acute Mountain Sickness (AMS) and its severe forms.<\/p>\n<p><strong>Being in good clinical, physical, and psychological condition allows for more efficient acclimatization and better overall performance. Make sure to gather information before venturing into a new environment for the first time.
